The practice will be moving back to the former premises at 72 Gordon Street, Burton-on-Trent DE14 2JA on Monday 24 March.
You do not need to do anything – you will remain registered as a patient at Gordon Street Surgery.
If you have a face-to-face appointment booked for or after Monday 24 March, this will still go ahead but at the address above.
